Application Process

We welcome your interest in Mayfield Junior School and look forward to working with you throughout the application process. The outline below provides an overview of the steps required to complete the admissions process.

Step 1: Inquire

To learn more about Mayfield Junior School and begin the admissions process, please complete an online inquiry. The information requested on the form allows us to open an applicant file for your child and will help streamline the process.

Step 2: Apply

The online application requires a username and password. An email with log in information will be sent to you after your inquiry is processed. Once you have completed the application your login credentials will also provide access to your individual application checklist. This allows you the opportunity to see what supporting documentation has been received or is still outstanding.

Step 3: Assessment and Interview

Kindergarten Applicants:
  • Kindergarten Readiness Visit
  • Parent/Guardian Interview
Grades 1-4
  • Academic Activity Session
  • Campus Visit
Grades 5-8
  • Student Assessment and Interview
  • Campus Visit

Step 4: Submit Supporting Documents

Supporting documents for all applicants must be received by January 16, 2024. Many of the required forms are available for download here or on the admissions checklist. 

  • Teacher Evaluation form (Requested through student admissions checklist)
