Application Process

We welcome your interest in Mayfield Junior School and look forward to working with you throughout the application process. The outline below provides an overview of the steps required to complete the admissions process.
At this time, we are continuing to accept applications in grades where there are openings. Please contact Lauren Marlis, Director of Enrollment: with any questions you may have.

Step 1: Inquire

To learn more about Mayfield Junior School and begin the admissions process, please complete an online inquiry. The information requested on the form allows us to open an applicant file for your child and will help streamline the process.

Small group campus tours will be available on Tuesdays and Wednesdays this fall. More information and registration links will be posted here later this summer.

Step 2: Apply

To access our online application you will need a username and password. Please contact the Admissions Office at 626.796.2774 or with your request. A document with log in instructions is available here. Once you have completed the application your login credentials will also provide access to your individual application checklist. This allows you the opportunity to see what supporting documentation has been received or is still outstanding.

*Early Kindergarten deadline: December 17, 2021
Grades K-8: January 18, 2022

*Preferred date for K applicants

Step 3: Assessment and Interview

Kindergarten Applicants:
  • Virtual Kindergarten Readiness Visit
  • Parent Interview
Grades 1-4
  • Virtual Academic Activity Session
Grades 5-8
  • Virtual Student Assessment and Interview
  • Character Skills Snapshot (Grades 6-8)

Step 4: Submit Supporting Documents

Supporting documents for all applicants must be received by January 18, 2022. Many of the required forms are available for download here or on the admissions checklist. 

  • Teacher Evaluation form (Requested through student admissions checklist)
  • Copy of birth certificate
  • Copy of baptismal certificate (Catholic applicants)
