Step 1: Inquire
To learn more about Mayfield Junior School and begin the admissions process, please complete an online inquiry. The information requested on the form allows us to open an applicant file for your child and will help streamline the process.
Step 2: Apply
To access our online application you will need a username and password. Please contact the Admissions Office at 626.796.2774 or admissions@mayfieldjs.org with your request. A document with log in instructions is available here. Once you have completed the application your login credentials will also provide access to your individual application checklist. This allows you the opportunity to see what supporting documentation has been received or is still outstanding. *Kindergarten application preferred deadline: December 15, 2022
