Field trips are a vital part of our curriculum, and we are grateful for the support of our parent and guardian volunteers who help make these experiences possible.

To ensure the continued safety and well-being of all students, we are updating our requirements for chaperone volunteers. All individuals wishing to chaperone a school field trip must now have the following four items along with a copy of your drivers license on file with the school office before the date of the trip:

• A current TB test result
  • Fill out the questionnaire linked on this page. If the answers are NO, provide to Nurse Evanjeli. If the answers are YES, please go to St. George’s clinic for additional testing.
• A completed Live Scan background check.
  • Mayfield will share the cost of the scan (Parent cost $40)
• A current VIRTUS training certification

• Mandatory Reporter Certification
  • If you have a current certification, please send to bwagner@mayfieldjs.org 

These requirements are a critical part of our commitment to providing a safe environment for our students. If you plan on chaperoning this year, please ensure all three of these items are up-to-date and have been submitted to the school office. We recommend completing these steps well in advance of any upcoming field trips.
